Don't you have access to the schematics for the game? Showing main pcb, monitor and all that good other stuff. Just looking at your attached info I think the 21v is only used for the sound amplifier.
Just a guess, but I doubt that the main board is booting, have you checked for a good 5v on it? Leave the monitor unplugged and try the voltage readings then. The monitor is a heavy current draw. Remember, there are TWO +12 volt power sources on the power supply board. The little regulator that stands up vertically and has a tiny heatsink attached merely operates the 12 volt light bulbs (coin entry bulbs, pushbuttons, and remove/throw lights). The larger regulator with the large heatsink provides power to the monitor.
The other large regulator next to the bridge rectifier is the +5 regulator.
